Billing Policies
Please be prepared to pay your co-pay at the time of service.
In order to bill your health insurance, it is the patient’s responsibility to make sure we have accurate demographic and insurance information. Please bring your insurance card to every visit and notify the office immediately if your home address has changed.
If you have a high-deductible plan and cannot pay your balance in full, we understand and will help you set up a payment plan. Please call the billing office at 410-647-6907.
Payments can be made via AthenaHealth Patient Portal, calling the office at 410-647-8300 or 410-647-6907, or by following the directions on your billing statement.
Mask Guidelines
We ask patients and family members to wear a mask
if you have a fever and/or a cough.
Camp/School/Sport Form Completion Guidelines & Fees
Your child must have had a well check in the last 2 years.
Please allow 2-3 days for form completion. We are unable to call when they are ready, but feel free to call the office to check on their status.
Forms can be dropped off at the front desk during office hours or placed in the FORMS box located by door 206. If you have an upcoming well check, bring the forms with you.
All forms must have the patient’s name and date of birth written at the top. Also indicate if you want to pick them up at the office, otherwise we will scan them into the patient portal.
FEES - $15 for each set of forms/child. $30 for same day request.
Vaccination Policy
Our physicians are vaccine-positive and strictly follow the immunization schedule recommended by the American Academy of Pediatrics.
Late Cancellations or No-Show Fees
You will receive courtesy reminders for well checks at least 48 hours prior to the appointment via text, email and phone. If you are not receiving reminder calls, please call the office or check the Athena patient portal to make sure we have the correct phone and email contact info.
If you cancel a well check or follow up appointment on the same day as the appointment OR No Show, you will be charged a $75 cancellation fee.
After Hours Coverage
Dr. Hackett and Dr. Ginsburg (SPDOCS) are pleased to announce that they have partnered with Hello Pediatrics for after hours coverage.
Hello Pediatrics is a group of Board-Certified Pediatricians that provide 24/7 after-hours clinical support* and offer the option of telehealth visits that could save you a trip to urgent care and are billed to your health insurance. Your child's visit notes will be forwarded directly to our clinical staff who will offer follow-up and continuing care as needed.
